Donations to either City Hospital Foundation or Jefferson Health Care
Foundation may be made in honor of, or in memory of, a friend or family
member.
University Healthcare patients and their families often state how
grateful they are for the compassionate care we deliver. You may choose
to show that gratitude by making gifts in honor of care givers, in
honor of a loved one cared for at one of our medical center, or in
memory of a loved one at the time of their passing.
Gifts may be made by check or credit card (Visa or MasterCard). Checks
should be made payable to the foundation of your choice and mailed to:

Honorials – please indicate the individual you are recognizing.
If they are one of our care givers, please indicate their department or
unit. If they are someone you simply wish to honor in a special way,
please provide an address so we may notify them of your kindness. The
amount of your gift will not be mentioned.
Memorials – please indicate the name of the deceased individual,
as well as a name and address of the loved one to be notified of your
contribution. The amount of your gift will not be mentioned.

All donations are tax-deductible as allowed by law and are used to
support patient care services at the medical center supported by the
foundation of your choice.
